Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.
The Weekly Driver analysis

Toyota was reviewing software for the Japanese market when the bench testing turned up two ways the backup camera could fail. Fourteen months later the finding had become a recall covering more than a million vehicles in the United States.

Campaign 25V744000, filed October 30, 2025, covers 1,024,407 Toyota, Lexus and Subaru Solterra vehicles equipped with the Panoramic View Monitor system, spanning 39 model entries and five model years. Owner letters went out January 2, 2026.

The Issue

The Panoramic View Monitor runs off a Parking Assist ECU, and the software in that unit has two distinct faults.

The first is a freeze. If the driver selects reverse within a specific window after switching the ignition on, the rearview image can freeze briefly during the backing event. The picture on the screen stops matching what is behind the car.

The second is a blank screen. If the ignition is turned on and off within a specific window, the rearview image may not display at all on the next start.

Toyota traced both to the image drawing process. Interruptions during that process could freeze the image, and camera optical alignment data could become abnormal while being written, which is what produces the blank screen on the following ignition cycle.

NHTSA cites failures against four separate paragraphs of FMVSS 111: S5.5.5, S5.5.6, S6.2.5 and S6.2.6. Those paragraphs govern how quickly the image must appear, how long it must persist, and the requirement that the system default to a compliant rearview image at the start of every backing event.

The Parking Assist ECU comes from Denso International America, listed as a Tier 1 OEM supplier.

NHTSA's filing lists 39 model entries. The campaign reaches nearly the entire Toyota and Lexus lineup for those years, along with the Subaru Solterra, wherever the vehicle was built with the Panoramic View Monitor system.

Only vehicles fitted with the Panoramic View Monitor are in scope. A base-trim car with a conventional single rearview camera is not part of this campaign even if the nameplate appears on the list, which is why the VIN lookup is the only reliable check.

Chronology and Investigation

Between April and December 2024, Toyota reviewed the Japanese-market Panoramic View Monitor software as part of unrelated work, prompted by field reports in Japan of images freezing and images failing to display.

Bench testing confirmed both mechanisms. Interruptions during the image drawing process could freeze the picture. Abnormal camera optical alignment data written by the Parking Assist ECU could prevent the image from appearing on the next ignition.

The campaign was filed with NHTSA on October 30, 2025, and owner notification followed on January 2, 2026.

Reported Incidents

Toyota's filing reports no crashes, injuries or fatalities connected to the defect. Like most FMVSS 111 campaigns this is a noncompliance recall rather than a defect recall driven by field harm.

The scale is what makes it notable. At 1,024,407 vehicles it is larger than the 844,027-vehicle Stellantis rearview camera campaign filed in August 2026, and the two describe closely related failures on unrelated hardware from different suppliers.

What Owners Should Do

Owner letters were mailed January 2, 2026. The remedy is a parking assist software update performed free of charge by a dealer.

Because this campaign spans four model years of used inventory, a car bought secondhand may never have had its letter forwarded. Run the VIN.

Symptoms to Watch For Before the Camera Fails

Both faults are tied to ignition timing, which makes them reproducible in a driveway.

Select reverse immediately after starting. The freeze fault appears when reverse is selected within a specific window after ignition on. Backing out straight after a cold start is the condition that triggers it.

Cycle the ignition quickly, then check the screen. Switching off and back on within a short window is what produces the blank screen on the following start.

An image that is stale rather than absent. A frozen picture is more dangerous than a black one, because it looks like a working camera. If the image does not change as the car moves, it has frozen.

Surround-view angles that do not match. Abnormal camera alignment data can show up as stitched views that no longer line up before it shows up as a missing image.

How Long Will the Recall Repair Take?

A parking assist software update is a short dealer appointment, well under an hour of bay time, with no parts required.

The constraint on a campaign this size is appointment availability rather than labor. A million vehicles moving through Toyota, Lexus and Subaru service lanes is why owner notification came two months after filing.

Bottom Line

A backup camera that freezes is worse than one that goes dark, because a frozen image still reads as live. That is the case for booking this one rather than waiting, even though no crashes have been reported against it.

The repair is free, quick and software-only. Run the VIN, and until it is done, use the mirrors and a shoulder check when backing out of a space with foot traffic behind you.
